1.  ECO KEIKAKU will send our specific documents that is “Notification of individual information purpose of use and Indication bill”, ” Individual information correction bill”, and Individual information suspension bill”, “The letter of attorney” to the person or agent.

2. Please send the document described the Contents, which is explained above. In this regard, please enclose document of personal identification.

3. With indication claim, Eco will send individual information disclosure notification.

4. Please acknowledge that we could not response the request such as indication claim by establishing of the low.

・In case of the person or minor or adult guardian’s legal representative, please submit following document with the document of personal confirmation.

・The documents that confirm it is legal representative himself.
(Two of these documents which is Driving license, Passport, Family register, Health insurance card, National pension pocketbook, other public document which can confirm the person ＊In case of the document which has photograph such as driving license and passport, please submit either.)
